Support Coordinator

As a NDIS Support Coordinator, your primary responsibility is to assist NDIS participants in achieving independence in all areas of their lives. This involves identifying their individual needs, identifying appropriate services and support to meet their goals, and helping them access these resources. You will work closely with participants to coordinate and implement support to ensure they are getting the most out of their NDIS plans and overcoming any barriers that may be present.
In this role, you will also be responsible for building strong relationships with participants, their families, and external service providers. You will help participants negotiate with providers and understand the costs associated with their NDIS budgets. You will ensure that service agreements and bookings are completed and take a holistic approach to capacity building, helping participants to address and overcome any barriers they may face.
The ideal applicant will have extensive experience in the disability sector, with a excellent knowledge of the NDIS and its processes.
Requirements:
- Certificate IV in Disabilities or Mental Health; or Bachelor degree in Social Work, Psychology or similar field
- NDIS Worker Screening check clearance
- NDIS worker orientation module
- Drivers Licence and vehicle
