What is Support Coordination?

Support Coordination helps you understand and implement your NDIS plan so your funded supports work effectively in everyday life.

A Support Coordinator can help you connect with disability, community, health and other mainstream services. They can assist you to compare providers, coordinate support arrangements and respond to barriers or changes.

Good Support Coordination does more than arrange services. It helps you make informed decisions, exercise choice and build the confidence to manage your supports more independently.

Yes. Participants generally have choice over which provider delivers their Support Coordination. Check any notice requirements in your existing service agreement before changing providers.

Home and community visits are available where appropriate. We can also arrange meetings by telephone, video call or at one of our offices.

Yes. We can work with Agency-managed, plan-managed and self-managed participants where the appropriate Support Coordination funding is included in the plan.
