Our Support Coordinators will support you in understanding and implementing the funded supports in your plan. What is Support Coordination?

Support coordination is a process that helps individuals with disabilities access the services and resources they need to achieve their goals and live independently. This can include assistance with finding and coordinating services such as therapy, transportation, housing, and employment.

There are three levels of support coordination that can be included in your plan:

Specialised Support Coordination

Generally delivered in a time limited, outcomes focused manner and by an appropriately qualified and experienced practitioner to meet the individual needs of the participant’s circumstances. Necessitated by specific high complex needs or high level risks in a participant’s situation, to reduce complexity in the participant’s support environment in the context of broader systems of support, whilst also assisting the participant in connecting with NDIS supports, negotiating solutions with multiple stakeholders and building capacity and resilience.

Support Coordination

Further qualifications/experience are required to strengthen a participant’s ability to design and build their supports with an emphasis on linking the broader systems of support across a complex service delivery environment. Coordination of Supports is to focus on supporting participants to direct their lives, not just their services. This may include resolving points of crisis, and developing resilience in the participant's network.

Support Connections

Assistance for participants to implement their plan by strengthening the ability to connect with the broader systems of supports and understand the purpose of the funded supports and participate in the community. Support Connection will assist a participant in understanding their plan, assisting in ongoing management of supports, and answering questions as they arise.

Why is Support Coordination Important?

Support coordination is essential for individuals with disabilities because it helps them navigate the complex system of services and resources available to them. Without support coordination, individuals may struggle to access the services they need and may not be able to fully participate in their communities.

How can a Support Coordinator help me?

A support coordinator is your own personal NDIS expert. Our staff have extensive experience in disabilities and have a great amount of local knowledge of services in the Hunter Region, including Newcastle, Lake Macquarie, Central Coast, Port Stephens, Maitland & Cessnock.

Your Support Coordinator (COS) will assist you in negotiating with providers about what they will offer you and how much it will cost out of your plan. Support coordinators will ensure service agreements and service bookings are completed. They will help build your ability to exercise choice and control, coordinate services, and access your local community.

They can also assist you in planning ahead to prepare for your plan review.

Support coordinators will assist you in optimising your plan, ensuring that you are getting the most out of your funded supports.

Depending on your individual goals, plan objectives and aspirations you may receive funding for these supports in your plan. Hunter Community Hub is able to provide support coordination at all levels.
