At Hunter Community Hub, we support people with disability to live comfortably and independently in their own homes. Our In-Home Care services provide practical assistance with everyday tasks, personal care, and ongoing support so participants can maintain control of their daily lives while remaining in familiar surroundings.
Our approach focuses on respect, reliability, and independence. Support is tailored to each participant’s goals, routines, and preferences, ensuring that assistance enhances independence rather than replacing it.
Personal Care Support
Personal care assistance is provided respectfully and with sensitivity. Our trained support workers help participants maintain health, hygiene, and comfort while preserving dignity and privacy.
Support may include:
- Showering, bathing, and grooming
- Dressing and personal hygiene
- Toileting assistance
- Mobility assistance and transfers
- Medication reminders
- Assistance with eating and drinking
Where appropriate, support is delivered according to individual care plans and health guidance.
Household Assistance
Keeping a home organised and comfortable can become challenging when living with disability. Our household assistance supports participants to maintain a safe and welcoming living environment.
Support may include:
- General cleaning and tidying
- Laundry and linen changes
- Dishwashing and kitchen cleaning
- Meal preparation and cooking support
- Grocery shopping assistance
- Household organisation
The goal is to ensure the home remains safe, comfortable, and manageable.
Supported Independent Living (SIL)
For participants who require regular or ongoing assistance at home, we provide Supported Independent Living (SIL) supports.
SIL focuses on helping participants build skills and maintain independence while receiving the level of support they need to live safely.
Support may include:
- Daily living assistance
- Personal care and hygiene support
- Household task support
- Skill development for independent living
- Overnight or regular support as required
- Assistance with routines and planning
SIL supports are provided in accordance with each participant’s NDIS plan and goals.
Daily Living Support
Our support workers assist with many day-to-day tasks that make living independently easier.
Examples include:
- Assistance with appointments
- Transport to community activities
- Support with hobbies and interests at home
- Help managing daily routines
- Support with communication or organisation
These supports help participants maintain independence while staying connected to their community.
Flexible and Person-Centred Support
Every person’s needs are different. Our supports are designed to be flexible and responsive, whether you require:
- A few hours of support each week
- Regular daily assistance
- Ongoing in-home support
- Complex support coordination between services
